    I would argue that often it goes further. If you are fast and loose with the “big D,” people remember you. The kind of people you don’t want to remember you. Because they will come in while you are working. I have had several instances where I have flat out told patients that I will not give them IV narcotics. IM? sure. Orals? Sure. I will also order any test for them in the book, up to and including an emergent MRI (yeah, I went there), but under no circumstances will I give them IV narcotics until I find some objective pathology which warrants it. A number of them simply leave at this point.
    It sickens me how manipulative these patients are. They are fundamentally holding your higher angels hostage. They are using our virtues of mercy and compassion against us.
    Makes me sick, I tell you.
